User Access Request-Change: Download and complete this form to request additional users to receive a CORA username and password for access to your facility outcome data.
New Providers
To begin utilizing CORA’s unique benchmarking reports and clinical resources:
1. Send an email to
data@coraonline.net requesting an enrollment
package be sent to your attention. In your request, please include:

Facility Name(s) & Address(s)*

Contact Name & Title

Contact Phone & Fax
2. Upon receipt of the CORA Enrollment Package:

Review and sign the CORA Data Use Agreement

Complete the Facility Profile

Fax both documents back to CORA at 631.841.6184.
If you do not receive the CORA Enrollment package within 3 days,
please call 866.340.2672.
IMPORTANT: CORA Laboratory Data Submission
Upon receipt of your completed enrollment documents to CORA, you will receive a CORA Laboratory Data Transfer Setup Instructions form. You will need to contact your laboratory services provider to request that your clinic(s) be added to the CORA interface. In addition, you may request that 3 months of laboratory results be transmitted initially to CORA for benchmarking and trending purposes.
Very Important: Your ability to access CORA is not confirmed until you receive a username and password from the Data Services Manager at CORA. Please call CORA if you have not received your username and password within 1 week of faxing all the required documents.
*For multiple facility enrollments, a list of associated clinics with Facility Name, Address, City, ST, Zip, Contact Name and Phone Number may be sent to expedite contract alignment.
